Please complete the registration form (pdf) included on this web site. Print and mail/fax the form with your check or credit card information to: MPhA Annual Convention, 650 W. Lombard Street, Baltimore, MD 21201-1572. Our FAX number is 410-727-2253. Register before the deadline and save the $40 on-site fee. The deadline for Advance Registration is Friday, May 23, 2008. | ||||||||||||||||||||||||||||||||||||||||||||||||||||||||
| On-Site Registration | Registration forms received after the advance registration deadline will not be processed. You will need to register when you arrive at the Clarion and add the $40 on-site fee. | ||||||||||||||||||||||||||||||||||||||||||||||||||||||||

To cancel your registration and receive a refund, a written request must be received by Friday, May 23, 2008. Cancellation requests received by this date will be processed and a refund issued less a $25 processing fee. Please allow 4-6 weeks for processing. | Hotel |
The Clarion Resort Fontainebleau is offering discounted hotel rates for MPhA attendees. To reserve your room, call the Clarion directly at 1-800-638-2100. You must tell the reservation agent you are with the Maryland Pharmacists Association to receive the special rate. A deposit equal to one night's stay is required to hold the reservation. Currently a 10.5% state and local tax is applicable to the room rates. The deadline is Wednesday, May 21, 2008. | ||||||||||||||||||||||||||||||||||||||||||||||||||||||||
| Hotel Cancellation |
Reservation cancellations must be made 72 hours prior to scheduled arrival to avoid a full room rate "No Show" charge. If you cancel, you must have a cancellation number to receive a refund.
