URGENT!!
Apply for Your National Provider Identifier Today!

If you have not applied for your National Provider Identifier (NPI), it is urgent that you do so without further delay!   
Be sure to include your “PHARMACY” taxonomy code, which begins with “33”.

Until March 1, 2007
The National Council for Prescription Drug Programs (NCPDP) has a deadline date of March 1, 2007 to receive your NPI.  This ensures that NCPDP is able to distribute your pharmacy’s NPI among the industry to avoid possible payment disruption after May 23, 2007.

• The NCPDP and NPI Pharmacy Application Form is available for download at http://www.ncpdp.org/frame_news_npi-info.htm. Be sure to attach a copy of your email or letter from CMS informing you of your pharmacy’s NPI, and fax with the completed form to NCPDP at 480-767-1043.

After March 1, 2007
If you have not applied for your NPI or notified NCPDP of your NPI, there is no guarantee Express Scripts will receive your NPI prior to May 23, 2007.

Follow the instructions above to notify NCPDP of your NPI, and please fax your NPI email or letter from CMS directly to Express Scripts at 866-735-8559 or email it to ESI at ProviderOutreach@express-scripts.com as soon as you receive it. 

If Express Scripts does not receive your NPI prior to May 23, 2007, when it is mandatory that we only accept the NPI on submitted claims, your claims will reject until we receive your NPI.
