Paid Pilot for Rural Maryland Pharmacies: Connect to CRISP and Build a Chronic Care Service
Pharmacy Owners:
We’re recruiting 20 community pharmacies in rural Maryland for a state-funded pilot, and we’d like yours to be one of them.
The Maryland Pharmacists Association (MPhA) Foundation has been awarded Rural Health Transformation Program funding from the Maryland Department of Health for the Maryland Interoperable Pharmacy Initiative (MIPI). This is an award offer pending a final executed agreement between MPhA Foundation and Maryland Department of Health. See Steven’s Amendment Disclosure below.
MIPI connects rural pharmacies to CRISP, Maryland’s health information exchange. That means you’ll be able to see your patients’ discharge summaries, lab results, problem lists, and care plans, and send your own documentation back to their other providers.
What your pharmacy receives via the grant
- A pharmacy-specific clinical EHR that runs alongside your dispensing system and connects to CRISP, paid for by the project
- Hands-on training and clinical coaching from faculty from one of Maryland’s Schools of Pharmacy and/or Maryland P3 Program, plus site visits and a peer learning group
- Ramp-up incentive payments of $45,000 for the grant year to support non-clinical staff time
- A spot in the group building payer contracts so these services get paid for after the grant ends
What we ask of your pharmacy
- Complete CRISP onboarding by March 30, 2027 (participation agreement, privacy and SUD attestations, lab data agreement, training, and a patient panel every 90 days)
- Train at least one pharmacist on the new workflow
- Choose diabetes, hypertension, or both, and enroll 10 patients by June 30, 2027
- Document your care in the EHR and share outcome data for the project report
Eligible pharmacies are in Allegany, Carroll, Frederick, Garrett, Washington, Cecil, Dorchester, Somerset, Talbot, Wicomico, and Worcester counties.
